Parental Consent
For your child to be included on photos for competitions or other achievements, a parental consent form needs to be signed by a parent or guardian. If you need another copy to print out, click on 'Consent Form' above (document is in Microsoft Word 'doc' format). If you are happy with the conditions please sign it and return the form to an instructor.

Membership
Licence Application forms and Licence renewel forms will soon be available to download from here. Send your licence form, with a self-addessed envelope inside to:

...and your licence will be mailed back to you. Each license lasts for one year. Make sure you keep upto-date by renewing your licence, which provides you with insurance cover in the event of an injury (detailed on the insurance slip in your licence book).
